Information About My Practice

RATES:

Individual Therapy:  $200.00 for a 50-minute session

Couples Therapy:    $250.00 for a 90-minute session.

Group Work:  Rates vary

PAYMENT INFORMATION

Payment is due in full at each session. I accept cash, check and credit card payments.

INSURANCE INFORMATION

I do not accept any insurance plans directly. All services are provided on a fee-for-service basis meaning that patients are responsible for full payment of fees.

If you have a health insurance policy, it will usually provide some coverage for mental health treatment. It is very important that you find out exactly which mental health services your insurance policy covers if you wish to submit a claim for reimbursement. Authorization is often required in advance. I will provide you with a super bill that you can submit to your insurance.

To determine if you have mental health coverage please call your insurance company.

Here are some questions you can ask:

  • What are my mental health benefits?
  • What is the coverage amount per therapy session?
  • How many therapy sessions does my plan cover?
  • How much does my insurance pay for an out-of-network provider?
  • Is a referral or approval required from my primary care physician?

 

CANCELLATION POLICY:

48 hours notice is required to reschedule or cancel your appointment.  Otherwise, you will be charged the full fee for your missed session. Please leave your message at 970. 317-5731.

BEFORE YOUR VISIT:

I use a practice management software system that supports both remote (HIPAA-compliant) and in-person sessions. Once scheduled you will receive and complete initial forms electronically prior to meeting.
